Choice Plus Plan
If you are enrolled in the UHC Choice Plus Plan, UHC Choice Plus HDHP Standard Plan, or UHC Choice Plus HDHP Premium Plan, you will need to search for a provider using the Choice Plus network:
Harvard Pilgrim Choice Plus Plan
If you are enrolled in the UHC Choice Plus Plan, UHC Choice Plus HDHP Standard Plan, or UHC Choice Plus HDHP Premium Plan and you also reside in MA, ME or NH, you will need to search for a provider using the Harvard Pilgrim network:
Options PPO Network
If you are enrolled in the UHC Choice Plus Plan, UHC Choice Plus HDHP Standard Plan, or UHC Choice Plus HDHP Premium Plan and you also reside in Utah, you will need to search for a provider using the Options PPO network:
