Transferring care

If you are new to UnitedHealthcare, there’s some important information and steps to consider before your plan goes into effect.

To help make your care transfer experience easier, start with this checklist of considerations:

  • Fill all open prescriptions with your current provider prior to your plan’s start date
  • Initiate a transfer of your prescriptions to a new network pharmacy if necessary
  • If switching providers, request a transfer of medical records prior to your start date
  • Consider whether you’ll need to continue active treatment for a chronic or acute condition
