Simplifying how special needs support works.

The Optum Special Needs Navigator program is designed to provide support for your child with special needs, including: ADHD, autism, eating disorders, substance use disorders, mood disorders, like depression and anxiety, developmental delays and more.

When you participate in the program, you’ll receive ongoing support from a navigator who can connect you with available services and providers, like:

  • Benefit explanation and consultation
  • Medical and/or behavioral health providers
  • Physical, occupational or speech therapy
  • Schools, local support groups and more

 

This program should not be used for emergency or urgent care needs. In an emergency, call 911 or go to the nearest emergency room. This program is not a substitute for a doctor’s or professional’s care. Due to the potential for a conflict of interest, legal consultation will not be provided on issues that may involve legal action against Optum or its affiliates, or any entity through which the caller is receiving these services directly or indirectly (e.g., employer or health plan). This program and its components may not be available in all states or for all group sizes and is subject to change. Coverage exclusions and limitations may apply.

With 24/7 Virtual Visits, you can connect to a care provider by phone or video* through myuhc.com® or the UnitedHealthcare® app. Providers can treat a range of non-emergency health conditions — and may even prescribe medication as needed**.

Smoking Cessation Programs

If you elect medical coverage through Pfizer, you and your eligible dependents can participate in the Pfizer Smoking Cessation Program, which is administered by UnitedHealthcare. Your eligible dependents are those individuals for whom you have elected medical coverage through Pfizer.

Through this program, you can receive 100 percent reimbursement for eligible smoking cessation programs or courses of treatment, including:

  • Behavioral modification programs offered through Optum;
  • Over-the-counter treatments, such as transdermal nicotine patches (e.g., Nicotrol, Nicoderm or Habitrol) and nicotine gum;
  • Prescription medications, such as Chantix and Nicotrol; and
  • Alternative treatments, such as acupuncture (if the acupuncture is performed by a covered provider) or hypnotherapy.

Pfizer Smoking Cessation Program Claims
You will need to complete a Pfizer Smoking Cessation Program claim form and send it, along with your smoking cessation program or product receipt, to UnitedHealthcare, at the address listed on the form, regardless of the Plan option or managed care plan in which you are enrolled. You can access a Pfizer Smoking Cessation Program claim form through Pfizer Benefit Center at http://www.netbenefits.com, or you can request a form by calling the Pfizer Benefit Center (see the “Resources” section in the back of this booklet).

Note: If you have waived medical coverage through Pfizer, you are eligible for the Smoking Cessation Program, as outlined above.

Completing a Claim Form
After obtaining a claim form for the Pfizer Smoking Cessation Program, follow the instructions on the claim form as carefully as possible in order to speed the processing of your claim. Be sure to answer all questions, include all required information and attach any required itemized bills to your form. To determine the status of a claim, contact UnitedHealthcare (refer to the “Resources” section in the back of this booklet).

How and When Claims Are Paid
Claims for the Pfizer Smoking Cessation Program should be submitted within 90 days after you incur an expense. Claims submitted after 90 days may be paid, depending on the reason for the delay. You may be asked to provide a detailed, written explanation.

Normally, benefit payment checks are sent to you. If you prefer, and your provider accepts assignment, the check can be sent directly to the provider. Make sure you complete the appropriate section of the claim form to assign benefits to a provider.

If you have a special condition, our Centers of Excellence (COE) network provides help finding a doctor and medical center as well as help with understanding your illness. COEs are intended to complement local hospital and physician care and offer benefits that may include: 

  • Higher rates of accurate diagnoses and appropriate therapy 
  • Fewer complications 
  • Planned and coordinated care provided by a team of experts 
  • Higher survival rates, shorter stays and decreased costs 
  • Expanded treatment options

Cancer Resource Services is here to help connect you and your family with dedicated cancer nurses who can offer personalized support during diagnosis through treatment. They will help you understand your diagnosis and share information that may help you make more informed decisions about your care as well as find emotional support for you and your family members.

Congenital heart disease resource services

Access a team of specialized Congenital Heart Disease nurses that will support you through all stages of treatment and recovery. They will help you make informed treatment decisions and guide you to top-performing Centers of Excellence providers.

Support for parents whose infants are admitted to the NICU

Neonatal Resource Services (NRS) provides a dedicated team of experienced neonatologists, neonatal intensive care unit (NICU) nurse case managers and social workers who can provide support and assistance during your infant’s admission to the NICU. The case manager will also provide discharge planning assistance and ongoing support post-discharge based on your infant’s needs.

The plan pays benefits under the Covered Health Care Service Category in your plan documents.
